Only for those who have recently had a session and decided to alter their script outside of the 2 week window. This is not a session, you will require a full follow up for a script renewal, this is only for an interim alteration.

Description

This is for script alterations, you have 2 weeks from the time of your session with the nurse practitioner to alter your script free of charge, if you decide to alter it outside fo the 2 weeks there is a fee charged by the NP.

This fee is for the NP to review your chart and alter your script according to your recent session. There is no session included in this. If you are looking for script renewal you MUST have appropriate blood work and a session with the nurse practitioner, you may do so here
