Reopen Chart – Nurse Practitioner ONLY

What is included:

A follow up script with necessary adjustments (medication fee not included), no supplement changes or additional recommendations as those require a session with Holly. You can add a session with Holly after your session with the NP or click here

Follow up labs will be necessary, the requisition will be less in depth than the first set as we will not need to test for as many items, this is customized based on your last lab workup and session with the NP. The follow up fee is for packages including but not limited to LDN therapy, hormone balancing, and thyroid. No matter the initial package the follow up is always under this particular category.

Session with the NP

There is no session with Holly in this package, if you would like to have a full follow up session with Holly as well please follow this link. You will qualify for her discounted existing patient rate for follow ups.

HWH will not use existing lab results, regardless of how recent they are, unless the nurse practitioner deems them satisfactory for assessment. This does not change the price of the package as there is not a fee charged by us for the requisition itself but rather the review of the results.
