In women who still menstruate, we shed the endometrial linking each month—think of it as a natural reset.

But underneath that lining is something called the endometrial stripe—and we don’t shed that with a regular period or even with a withdrawal bleed from birth control. 

Here’s the issue:

When progesterone is low (which is common in perimenopausal women and women with PCOS), the estradiol you’re making can build up the endometrial stripe—and the thicker it gets, the higher your risk of endometrial cancer.

What helps? Progesterone!

It’s not just about symptom control—it’s protective. It balances out the unopposed estrogen and prevents (as well as shrinks!!) an overgrown stripe. So why the heck aren’t more providers using it?

D&C or Progesterone, which one would you pick?

Yes, your can give you a D&C to “clean it out” but you’ll only end up needing another. How about an ablation or some birth control pills…. Or we can prevent it with progesterone (we can treat it with this as well). Wouldn’t that make more sense?
